This award winning, research-based program was designed to connect to and support family caregivers upstream from the need for full hospice services. Designed to provide information, dispel myths and misperceptions about hospice, decrease barriers to hospice, facilitate discussions, and help families support their loved ones through the last years of life with a focus on what is most meaningful and important to them. The toolkit provides skill building and training materials needed to engage family caregivers in your community. Resources and materials can also be used in day-to-day care of patients and families providing interdisciplinary team members with tools to support the psychosocial, spiritual, and relationship experiences of patients and families.
