This clinical reference tool was developed to provide clinical staff with assessment and care management references and guidelines for hospice, palliative care and home health patients. These handy reference cards can be used to address common problems clinicians encounter and offer suggested interventions. Suggested assessment and pharmacologic and nonpharmacologic interventions are provided for: Mouth Care/Oral Irritations, Nausea and Vomiting, Anxiety, Dyspnea, Bowel Complications, Terminal Agitation/ Restlessness and Excessive Respiratory Secretions. The laminated cards are small (7” x 4.5”) and sold on a ring for ease of use.
